5.1.5 Service Placements Process

The System for recording Service Placements is now electronic with the SS35 forms being sent via e-mail.  Please note there are two separate forms;  SS35 for movements eg started to be looked after, change of placement, end of LAC episode. The SS10 is for all other payments.

SS35

Social Workers are to complete the SS35 form electronically ensuring all details are recorded on the SS35. The social worker will send the e-mail to the following email addresses:-

  • SC_CF LAC  to allow LAC admin to enter the service placement onto RAISE.
  • Own Team Manager for information.

On RAISE, admin within the LAC Admin Support will:

  • Insert the Legal Status, Service Placement (including carers name and address in notes/comments field) and change the address. 
  • Send SS7 notifications to other authority etc.
  • Fax a copy to Finance
  • scan the SS35 into Comino.
  • Ensure that an active family home address remains on RAISE for reporting purposes.
  • A Schedule of Service is required for this type of placement and the service detail entered requires the "Ready to Authorise" box to be ticked.  Admin will also change the service placement status to ' authorisation request'.
  • LAC Admin will update the necessary dates for Health Assessments, Dental Appointments and SDQ's etc. (Emotional & Behavioural health questionnaire).
  • LAC admin support will e-mail locality admin to complete necessary documentation i.e. Health Assessments and SDQ's etc to pass to workers

Sandra Westgarth, or assigned deputy, to check the authorisation module in RAISE for "authorisation requests" for Agency Foster Placements or foster placements on a regular basis and action. 

Julie Chew, or assigned deputy, to check the authorisation module in RAISE for "authorisation requests" Adoption placements on a regular basis and action. 

Mark Nevill, or assigned deputy, to check the authorisation module in RAISE for "authorisation requests" for Residential Placements on a regular basis and action. 

**Unless a placement is authorised on RAISE a payment will not be initiated**

SS10 Should be completed as above but NOT sent to LAC.  Locality admin will continue process these forms.
